Cerebrovascular Risk in Chronic Glomerulonephritis: Clinical and Functional Implications

Studies of recent years found that, in more than half of people with pre-dialysis stages of chronic kidney disease, death occurs as a result of cardiovascular diseases.
Purpose of the study. Study of the prevalence of risk factors for cerebrovascular diseases in chronic glomerulonephritis (CGN) at pre-dialysis stage of the disease.
Materials and methods. 521 patients (351 men and 170 women) with CGN at pre-dialysis stage of the disease were examined. Mean age of the examinees was 37.7±11.7 years.
Results. Compared to the group of men, women showed significantly higher rates of tachycardia (67.6% vs. 28.4%, p<0.005) and stage II arterial hypertension (96.4% vs. 13.3%, p<0.001). In the examined cohort, the share of males with increased linear dimensions of the heart prevailed noticeably. Among female patients, such CVD risk factors as high level of C-reactive protein (41.1% vs. 22.7%, p<0.005), hyperfibrinogenemia (80.5% vs. 69.8%, p<0.005), anemia (40.5% vs 25.6%, p<0.005) were significantly more frequent; and there was a significant decrease in glomerular filtration rate [66 (37-86) ml/min vs. 85 (48-111) ml/min; p=0.000], compared to male patients.
Conclusions. In patients with chronic glomerulonephritis at pre-dialysis stage of the disease, high prevalence of risk factors for cerebrovascular diseases is observed. Leading positions belong to tachycardia, arterial hypertension and excess body weight. In females, anemia, hyperfibrinogenemia and high level of C-reactive protein that are associated with a significant slowing of the glomerular filtration rate are registered significantly more often.
