Comparison of the Effectiveness of Posterior Fossa Decompression without Dural Opening Versus with Extraarachnoid Duraplasty in Chiari Malformation Type I

Abstract

A comparative analysis was conducted to evaluate the effectiveness of two surgical techniques for Chiari malformation type I: posterior fossa decompression without dural opening (PFD) and with extraarachnoid duraplasty (PFDD). The study included 42 patients operated on between 2016 and 2024 at the Surgical Clinic of Azerbaijan Medical University and the Republican Neurosurgical Hospital. Outcome assessment using the Chicago Chiari Outcome Scale (CCOS) revealed a favorable postoperative result (≥13 points) in 70.8% of cases following PFD and in 77.8% after PFDD. Neurological symptom regression was more pronounced in the PFD group (95.8%), although some patients experienced recurrence of pain. Complications (CSF leakage, infection) occurred only in the PFDD group. The findings demonstrate comparable effectiveness of both techniques, highlighting the importance of anatomical stratification in selecting the optimal surgical approach.
